Key result
Simultaneous use of actigraphy and ambulatory blood pressure monitoring enables precise, clock-independent calculation of nighttime blood pressure and detection of sleep-related blood pressure changes.
Why the study?
A disturbed diurnal blood pressure profile is a key risk factor for cardiovascular diseases, prompting analysis of simultaneous ambulatory blood pressure monitoring and actigraphy to aid interpretation in clinical decision-making.
Does simultaneous actigraphy improve the accuracy of diurnal blood pressure profile evaluation in patients undergoing ABPM?
Does simultaneous actigraphy improve the accuracy of diurnal blood pressure profile evaluation in patients undergoing ABPM?
Adding actigraphy to ABPM allows for objective, clock-independent determination of sleep and wake periods, leading to more accurate assessment of nocturnal blood pressure dipping and cardiovascular risk.
Combined ABPM-actigraphy may refine profile interpretation in select cases; leaves open prospective outcome validation before routine adoption.
A disturbed diurnal blood pressure profile is one of the most important risk factors of cardiovascular diseases. This review analyzes the use of simultaneous diurnal ambulatory blood pressure monitoring (ABPM) and motion activity monitoring (actigraphy) to obtain additional information for correct interpretation of ABPM results in clinically significant decision-making. The article considers practical aspects of actigraphy in expert ABPM for clock-independent calculation of the parameters of nighttime and daytime blood pressure (BP); detection of BP changes during sleep; connection with respiratory disturbances during sleep, motion activity, and body position; and sleep deprivation in shift workers. Original illustrations of simultaneous ABPM and actigraphy are provided.

Gorokhova et al. (2020) conducted a review in Arterial hypertension. Actigraphy combined with ambulatory blood pressure monitoring vs. Standard ambulatory blood pressure monitoring was evaluated. Simultaneous use of actigraphy and ambulatory blood pressure monitoring enables precise, clock-independent calculation of nighttime blood pressure and detection of sleep-related blood pressure changes.
