To assess the incidence of and risk factors for extended-spectrum β-lactamase–producing Enterobacteriaceae (ESBLE) infections following liver transplantation.
Screened 710 liver transplant recipients preoperatively for fecal carriage of ESBLE over a 10-year period (January 2001–April 2010) in France.
Tracked posttransplant ESBLE infections occurring within 4 months after surgery, characterized bacterial isolates via molecular typing, and evaluated clinical risk factors.
Overall, 5.5% of transplant recipients developed an ESBLE infection within 4 months post-surgery, with infection rates markedly higher among pretransplant fecal carriers than noncarriers.
Independent clinical predictors of posttransplant ESBLE infection were pretransplant fecal carriage, a Model for End-Stage Liver Disease (MELD) score >25, and surgical reintervention.
Typing demonstrated broad genetic diversity dominated by CTX-M enzymes, indicating that community-acquired carriage brought into the hospital contributes more to posttransplant infection epidemiology than nosocomial cross-transmission.
