Does residual pulmonary hypertension after TEER predict all-cause death or heart failure hospitalization in patients with secondary mitral regurgitation?
Residual pulmonary hypertension after TEER for secondary mitral regurgitation is an independent predictor of adverse clinical outcomes, highlighting the prognostic importance of post-procedural hemodynamics.

Residual PH may identify higher-risk secondary MR patients post-TEER; leaves open whether PH-directed therapy alters outcomes.
BACKGROUND: The effect of residual pulmonary hypertension (PH) on clinical outcomes in patients with secondary mitral regurgitation (MR) after transcatheter edge-to-edge repair (TEER) remains unexplored. METHODS AND RESULTS: We analyzed 62 patients with secondary MR who underwent TEER. In 32 (51.6%) patients with residual PH after TEER, adverse clinical events were observed more frequently compared with those without residual PH. Multivariate analysis confirmed residual PH as an independent predictor of all-cause death or hospitalization for heart failure. CONCLUSIONS: In patients with secondary MR, residual PH after TEER is an independent predictor of adverse clinical outcomes.

Tsubata et al. (2025) conducted an observational in Secondary mitral regurgitation (n=62). Residual pulmonary hypertension (mean pulmonary arterial pressure >20 mmHg) vs. No residual pulmonary hypertension was evaluated on Composite of all-cause death or hospitalization for heart failure (HR 3.99, 95% CI 1.41-11.3, p=<0.01). Residual pulmonary hypertension after transcatheter edge-to-edge repair was independently associated with a significantly increased risk of all-cause death or heart failure hospitalization (HR 3.99).
