Is transcatheter aortic valve-in-valve implantation feasible and effective for treating severe failure of a right ventricle-to-aorta homograft conduit in an adult patient with Fontan circulation?
This case demonstrates that transcatheter aortic valve-in-valve implantation in a heavily calcified right ventricle-to-aorta homograft is a feasible, less-invasive alternative to high-risk redo surgery in adults with Fontan circulation, though sequential valve implantation may be needed to resolve paravalvular leakage.

Supports feasibility of sequential valve-in-valve in Fontan patients with failing homograft; hypothesis-generating and should not yet change practice.
Catheter interventions can offer patient tailored solutions in high-risk congenital heart disease patients. A 21-year-old male with a Fontan circulation in the setting of unbalanced atrioventricular septal defect with a hypoplastic left ventricle and an aortic homograft connecting the right ventricular outflow tract to the ascending aorta, developed failure of the heavily calcified homograft with severe regurgitation and stenosis. He underwent three sequential transcatheter aortic valve-in-valve implantations to address the homograft failure and the subsequent paravalvular regurgitation, with satisfactory result and improved hemodynamics.

Nederend et al. (2023) conducted a case report in Fontan circulation with unbalanced atrioventricular septal defect, hypoplastic left ventricle, and failing calcified aortic homograft (n=1). Transcatheter aortic valve-in-valve implantation (three sequential 20 mm Edwards Sapien 3 Ultra bioprostheses) was evaluated on Homograft hemodynamic function, paravalvular regurgitation, and NT-proBNP levels at 3 months post re-intervention. Sequential transcatheter aortic valve-in-valve implantation reduced the homograft mean gradient from 42 to 22 mmHg and NT-proBNP from 785.8 to 173.9 ng/L at 3 months in a Fontan patient.
