We describe a patient with group G streptococcal endophthalmitis and review the salient clinical and epidemiologic aspects of 16 previously reported cases. Infection developed via both exogenous (generally via ocular surgery or trauma) and endogenous (all with bacteremia) routes. Outcome was poor with complete (75%) or partial (13%) visual loss and 1 death. Group G streptococcal endophthalmitis is a devastating ocular infection for which optimal antimicrobial and surgical management has not been precisely determined.
