In order that ejaculation may take place correctly, it is necessary that the vesical neck and the smooth sphincter within it contract violently in order to establish resistance against the seminal liquid flowing back into the bladder (an action effected through the nervous fibres of the erector). This is the only essential function of the internal smooth sphincter – not urinary continence, as was believed until now. In fact, when the sphincter is paralytic (Schramm) or is destroyed or surgically removed, no urinary incontinence is ever noticed; instead, only a vesical reflux of the spermatic liquid during the sexual acts is observed. This fact confirms that the smooth sphincter of the vesical neck is a genital organ.
