Persistent coronary microvascular dysfunction was found in 25% of post-PCI patients and was linked to impaired left ventricular contractile reserve and worse 1-year health status.
Why the study?
Does dipyridamole exercise stress echocardiography with speckle tracking identify coronary microvascular dysfunction and its impact on LV functional reserve in post-PCI patients?
Does dipyridamole exercise stress echocardiography with speckle tracking identify coronary microvascular dysfunction and its impact on LV functional reserve in post-PCI patients?
Dipyridamole exercise stress echocardiography with speckle tracking provides a safe, radiation-free method to assess coronary microvascular dysfunction, which persists in 25% of post-PCI patients and correlates with worse clinical status.
This prospective study combines dipyridamole exercise stress echocardiography (DExE) with two-dimensional speckle tracking echocardiography derived myocardial strain and work indices. It evaluates the influence of coronary microvascular dysfunction (CMD) on left ventricular (LV) functional reserve in patients with preserved LV ejection fraction after successful percutaneous coronary intervention (PCI) for left anterior descending (LAD) artery lesions. The authors find persistent CMD in a quarter of post-PCI patients and link this condition to impaired LV contractile reserve and worse 1 year health status. This work addresses a critical unmet need in noninvasive CMD assessment and provides a safe radiation free imaging protocol with high diagnostic sensitivity for microvascular ischemia. There are still several key knowledge gaps to address to maximize the clinical translation of this promising methodology.

Zhai et al. (2026) conducted an editorial in Coronary microvascular dysfunction post-PCI. Coronary microvascular dysfunction (CMD) was evaluated on Left ventricular contractile reserve and 1-year health status. Persistent coronary microvascular dysfunction was found in 25% of post-PCI patients and was linked to impaired left ventricular contractile reserve and worse 1-year health status.
