A man, aged 64 years, was admitted to hospital suffering from disseminated nocardiosis, manifested by a high fever, multiple subcutaneous abscesses, urinary tract involvement and the syndrome of inappropriate secretion of antidiuretic hormone. He responded rapidly to a trimethoprim-sulphamethoxazole combination. Though further evidence is clearly required, it is possible that this drug is the treatment of choice for nocardiosis.
