Nine renal allograft recipients developed prolonged fever and hematologic abnormalities during the first three months following transplantation. Pulmonary symptoms ("transplant lung") occurred in three patients and hepatic dysfunction in two during the clinical course. Cytomegalovirus (CMV) was incriminated in the etiology by the finding of viremia, viruria, and/or a rise in the CMV complement-fixation (CF) titer in each patient. Transfused blood administered during hemodialysis was suggested as the mode of transmission of CMV by the observation of an elevated CF titer in transfused blood given to two patients.
