Two patients with pancreatic carcinoma were found to have positive differential spot tests for mononucleosis (Monospot tests) early in the course of their disease. The Monospot test is considered highly specific for infectious mononucleosis with false-positive reactions reported only in acute leukemia, lymphoma, and infectious hepatitis. Liver biopsies in both patients showed no hepatic cell injury or mononuclear infiltrate, thus excluding the possibility of coexisting infectious hepatitis.
