The purpose of discharge planning is to provide chronic patients and patient' s family with continuing and complete health care. Therefore, the implementation of a continuing care system ought to be more systematic, meaning that with the cooperation of the patients, the patient's family, long-term car facilities and the discharge planner the continuing and complete health care system will be achieved and will meet the goal of discharge planning. This research adopts Quality Function Deployment to establish the continuing care system of discharge planning to search for the real needs of a patient's family and long-term care facilities. The findings of this research note that with regard to the long-term care of St. Joseph Hospital, the first-level matter is to provide a patient's family and long- term care facilities with a professional service, to strengthen the education of the patient's family, to help the patient return to hospital for diagnosis and to establish a single window of long-term care facilities. We hope that the results of this research can provide a reference for other hospitals in discharge planning.
