Key result
Vascular closure devices significantly reduced a composite of all vascular complications compared to manual compression or suture (10.4% vs. 17.2%; OR 0.70; 95% CI 0.50-0.98; p=0.04).
Why the study?
Vascular closure devices reduce time to hemostasis in single-point arterial access sites, but their benefit in electrophysiology procedures requiring multi-point venous access remains uncertain.
Do vascular closure devices reduce vascular complications and recovery time compared to manual compression or FO8 suture in patients undergoing EP procedures?
Meta-Analysis (n=848)
Do vascular closure devices reduce vascular complications and recovery time compared to manual compression or FO8 suture in patients undergoing EP procedures?
Vascular closure devices modestly reduce vascular complications and significantly shorten recovery times compared to manual compression or FO8 suture after electrophysiology procedures.

VCDs reduce hematomas and vascular complications while shortening recovery after EP procedures; supports broader adoption in multi-site venous access.
Background Vascular closure devices (VCDs) have been shown to reduce time to hemostasis in single-point arterial access sites, but their benefit in electrophysiology (EP) procedures requiring multi-point venous access remains uncertain. Objective To perform a systematic review and meta-analysis evaluating the efficacy and safety of VCDs compared with manual compression or figure-of-eight (FO8) suture for venous hemostasis following EP procedures. Methods A systematic search of PubMed, Scopus, and Google Scholar was conducted in June 2025 to identify randomized controlled trials (RCTs) comparing VCDs with manual or FO8 closure in EP procedures. Results Six RCTs including 848 patients (460 treated with VCDs) were analyzed. VCDs significantly reduced hematoma formation (3.4% vs. 7.7%; OR 0.51, 95% CI 0.29–0.90; p=0.02) and a composite outcome of all vascular complications (10.4% vs. 17.2%; OR 0.70, 95% CI 0.50–0.98; p=0.04). Time to hemostasis, ambulation, and disposition were significantly shorter in the VCD group by 5.5, 129, and 84 minutes, respectively. Conclusion VCDs modestly reduce vascular complications and significantly shorten recovery time following EP procedures.

Knijnik et al. (2026) conducted a meta-analysis in Electrophysiology procedures requiring venous hemostasis (n=848). Vascular closure devices vs. Manual compression or figure-of-eight suture was evaluated on Composite outcome of all vascular complications (OR 0.70, 95% CI 0.50-0.98, p=0.04). Vascular closure devices significantly reduced a composite of all vascular complications compared to manual compression or suture (10.4% vs. 17.2%; OR 0.70; 95% CI 0.50-0.98; p=0.04).
