A case of adenocarcinoma of the kidney with periaortic node involvement in a 10-year-old girl is presented. Nephrectomy and irradiation have produced an apparent cure as demonstrated by a 10-year follow-up. The number of children with documented adenocarcinoma of the kidney is now about 60, and this diagnosis should be considered when children present with gross hematuria or abdominal mass. The rarity of this disease in children may be accounted for by combination of a slow growing tumor preceded by a slow growing precursor (benign adenoma).
