Anterior segment optical coherence tomography (AS-OCT) has revolutionised corneal disease evaluation by providing high-resolution, non-invasive cross-sectional imaging. This technology enables precise visualisation of corneal microstructure, quantitative disease assessment, and objective therapeutic monitoring. Through a series of illustrative clinical cases, this article highlights the utility of AS-OCT in infectious keratitis, autoimmune corneal disease, corneal dystrophies, Descemet’s membrane detachment, lamellar keratoplasty, descemetocele, corneal perforation, hydrops, and post-refractive surgery complications. Each case demonstrates how AS-OCT findings complement clinical examination, guide surgical planning, and facilitate longitudinal follow-up. By bridging clinical presentation and anterior segment pathology, AS-OCT has become indispensable in contemporary cornea practice, enhancing diagnostic accuracy and optimising outcomes.
