A left-sided thoracotomy approach for Extent I thoracoabdominal aortic aneurysm repair may be a viable alternative to reduce surgical trauma in patients with complex abdominal conditions like giant ventral hernias.
Despite extensive experience, open thoracoabdominal aortic aneurysm repair is a technically complex procedure associated with high mortality and morbidity. Ones of the most common causes of postoperative complications are extended surgical access and surgical trauma. The main surgical approach for thoracoabdominal aortic aneurysm repair is thoracophrenolumbo-/laparotomy. We present Extent I thoracoabdominal aortic aneurysm repair through the left-sided thoracotomy in a patient with a giant ventral hernia. In our opinion, the new approach can significantly reduce surgical trauma and improve postoperative results.
