The authors report a single case of severe cerebrovascular spasm after the trans-sphenoidal removal of a pituitary adenoma. The patient died subsequently of the neurological sequelae caused by the vascular spasm and, at autopsy, was found to have residual tumor within the cavernous sinus as well as considerable evidence of ischemic brain damage from the spasm. The spasm was documented angiographically. This is, to our knowledge, the first such case reported in the literature. (Neurosurgery, 7: 382-386, 1980)
