Eighteen patients with end-stage renal failure secondary to primary bilateral vesicoureteral reflux are presented. These cases document a potential terminal phase in the natural history of vesicoureteral reflux. Five patients had sterile reflux alone, and four patients probably had sterile reflux until urinary tract infection occurred at our near endstage renal failure. The remaining patients had reflux associated with recurring urinary tract infection. Early diagnosis and proper management are important in vesicoureteral reflux.
