The Dhaman Insurance Scheme (DIS) provides health coverage to approximately 3.5 million expatriates in Kuwait, representing more than two-thirds of the population. Despite rising health care costs and inflation, DIS premiums have remained unchanged for nearly 25 years, raising concerns about the scheme's long-term viability. This study aims to evaluate the DIS by conducting a retrospective analysis of hospitalization records (2017-2022). The study highlights several challenges, including the high incidence of expensive medical and surgical conditions that place considerable pressure on the health care system. The study emphasizes the need for premium adjustments, enhanced data reporting and coding systems, and the development of a case-mix index to better inform resource allocation.
