Three patients with acute massive multilobar ischemic infarction with uncal herniation and impending death, who were unresponsive to conventional modes of therapy, were treated by ipsilateral hemicraniectomy. All three patients survived, although severe fixed neurological deficit persisted in two. The indication for hemicraniectomy in the management of patients with acute ischemic stroke is reviewed.
