In order to elucidate the mechanism of chlorpropamide antidiuresis in patients with diabetes insipidus, 4 patients were studied. Measurements of the half-life and effectiveness of a moderately large dose of antidiuretic hormone were performed during control and treatment periods. No changes in half-life or effectiveness were noted. It is suggested that chlorpropamide augments posterior pituitary release and synthesis of antidiuretic hormone, permitting osmolal control to be re-established in patients with diabetes insipidus. Chlorpropamide may affect similar intracellular mechanisms in both the beta cell and the supraoptic neuron.
