A case of pheochromocytoma diagnosed with 1-123 MIBG scintigraphy in a patient with bilateral polycystic kidneys is reported. Sonography was not diagnostic because of the presence of renal pathology, whereas CT identified the adrenal mass only after contrast enhancement. Computed tomography was unable to characterize it. MIBI scan showed focal accumulation in the left upper abdomen consistent with diagnosis of pheochromocytoma. A Tc-99m MAG3 scan defined the location over the upper left renal pole. I-123 MIBG may allow the correct localization and characterization of a pheochromocytomas when other imaging modalities are not conclusive.
