Why the study?
Severe mitral stenosis for non-cardiac surgery poses a challenge to anesthesiologists, as both general and regional anesthesia have limitations.
Graded segmental epidural anesthesia can be a viable option for laparotomy in patients with severe mitral stenosis, though post-operative heart failure may occur and require management with CPAP.
Graded epidural may be feasible for severe mitral stenosis in noncardiac surgery; leaves open need for controlled trials on anesthetic choice.
A BSTRACT Severe mitral stenosis for non-cardiac surgery is a challenge to anesthesiologists. General anesthesia as well as regional anesthesia have their own limitations. We present here a case of abdominal hysterectomy with ovarian cyst in a patient of severe mitral stenosis managed with graded epidural anesthesia who had heart failure in the post-operative period but was successfully managed by continuous positive airway pressure.
