Acute emotional stress linked to SCAD in a 51-year-old woman with multiple risk factors.
Why the study?
Differentiating SCAD from atherosclerotic ACS and Takotsubo cardiomyopathy is challenging when multiple risk factors are present, and important gaps remain regarding risk, triggers, and association with FMD.
Case Report (n=1)
Highlights the diagnostic challenge of identifying SCAD in patients with multiple cardiovascular risk factors and emotional triggers, given its overlap with atherosclerotic ACS and Takotsubo cardiomyopathy.
Accurate angiographic SCAD diagnosis remains essential to avoid inappropriate PCI; leaves open optimal strategies in patients with overlapping risk factors.
Spontaneous coronary artery dissection (SCAD), once considered rare, is increasingly recognized as a cause of acute coronary syndrome (ACS) owing to advances in coronary imaging. Although classically described in women with no or minimal cardiovascular risk factors, differentiation from atherosclerotic myocardial infarction remains challenging when multiple risk factors are present. Accurate diagnosis by coronary angiography is essential, because interventions such as percutaneous coronary intervention and thrombolysis may cause harm in SCAD. Diagnostic overlap with Takotsubo cardiomyopathy (TTC; also called stress cardiomyopathy) further complicates early evaluation. Important gaps remain regarding who is at risk, the triggers involved, and the association with fibromuscular dysplasia (FMD). We report a 51-year-old woman with multiple cardiovascular risk factors who developed SCAD two days after an acute emotional stressor, with negative FMD screening.

Buckner et al. (2026) conducted a case report in Spontaneous coronary artery dissection (SCAD) (n=1). Acute emotional stressor was evaluated on Spontaneous coronary artery dissection. An acute emotional stressor triggered spontaneous coronary artery dissection in a 51-year-old woman with multiple cardiovascular risk factors and negative fibromuscular dysplasia screening.
