Although Americans are paying increasing attention to the global HIV/AIDS epidemic, concern about the domestic epidemic is falling. However, with almost 400,000 American living with AIDS, the epidemic is far from over. Challenges to prevention include the need to increase the number of persons who know their HIV infection status, as well as increasing prevention efforts, especially among men who have sex with men and in the African-American community. To advance HIV prevention will require using new HIV testing technologies, identifying new prevention venues, taking actions that have not been politically acceptable, and identifying new leaders in affected communities.
