A retrospective study of twenty‐six patients with ‘non union’ of the femoral neck treated by fibular gone graft, with or without subtrochanteric osteotomy, over the past twenty years is discussed. These cases of ‘non union’ often result from unrecognized fracture, poor primary reduction, and/or inadequate internal fixation of the fracture. Preoperatively one should be reasonably certain of the viability of the femoral head, as evidenced by bone scan. Bone grafting is preferable to prosthetic replacement in patients with ‘non union’ of Intracapsular femoral neck fractures, except in the frail and the elderly.
