Summary: A 23‐year‐old woman, otherwise healthy, hurt her left knee in a fall with resulting superficial abrasions and pain. This was followed by inflammation and effusion and the pain continued. Because damage to the meniscus was suspected, she underwent diagnostic arthroscopy. Initially, only severe synovitis was detected. After multiple arthrocentesis and several surgical interventions, a severe gonarthritis caused by Scedosporium apiospermum was finally diagnosed 10 months after the trauma. The superficial abrasions may have provided the means of entry for the fungus. After antimycotic treatment with miconazol and itraconazol applied over a period of 10 weeks, the patient returned to her previous job as a nurse and has so far (6 years) remained without any further sign of infection.
