A surgical technique of tendon-sparing iliopsoas tunnel deepening with anterior labral refixation is presented for the management of iliopsoas impingement.
Why the study?
Traditional surgical management of iliopsoas impingement with lengthening or release raises concerns for flexion weakness and iliopsoas atrophy, prompting alternative techniques.
Describes a tendon-sparing surgical technique (iliopsoas tunnel deepening with anterior labral refixation) to manage iliopsoas impingement while avoiding flexion weakness and atrophy.
Describes tendon-sparing tunnel deepening for iliopsoas impingement; leaves open whether it reduces weakness versus release in hip preservation.
Iliopsoas pathology is commonly diagnosed when treating patients presenting with hip pain. Clinically associated with painful internal snapping in the setting of femoroacetabular impingement or acetabular dysplasia, iliopsoas impingement traditionally has been managed surgically with iliopsoas lengthening or release. Concerns about the development of flexion weakness as well as iliopsoas atrophy have prompted the development of additional techniques to manage psoas pathology during hip-preservation surgery. We present a technique of iliopsoas tunnel deepening with anterior labral refixation for the management of iliopsoas impingement.

Kuhns et al. (2025) studied Iliopsoas impingement and painful snapping hip. Tendon-sparing iliopsoas tunnel deepening with anterior labral refixation was evaluated. A surgical technique of tendon-sparing iliopsoas tunnel deepening with anterior labral refixation is presented for the management of iliopsoas impingement.
