Modern sulfonylureas such as glimepiride provide pleiotropic benefits beyond glycemic control in type 2 diabetes, including improved insulin sensitivity, increased adiponectin, and proven cardiovascular safety.
Why the study?
Modern sulfonylureas have evolved with extra-pancreatic pleiotropic benefits that extend beyond their known glucose-lowering effects.
Modern sulfonylureas provide significant extra-pancreatic pleiotropic benefits, including cardiovascular safety and anti-inflammatory effects, supporting their continued use in Type 2 diabetes management.
Supports modern sulfonylurea use in T2D; hypothesis-generating and requires prospective trials before practice change.
Department of medicine, Educare institute of dental science Malappuram, Kerala and SafeCare clinics, Tirur, Kerala sulfonylureas (SUs) are one of the oldest, time tested and most commonly used oral antidiabetic agent in Type 2 diabetes. Modern SUs like glimepiride and gliclazide XR were preferred over conventional SUs due to its better efficacy and minimal side effects. The use of Modern SUs in the treatment armamentarium of T2DM has evolved, over past few decades. Modern SUs possesses beneficial pancreatic glucose lowering effect and more interestingly extra-pancreatic pleiotropic benefits. This review article discusses on the utility and benefits of SUs beyond glycemic control and explaining on their pleiotropic effects.

B Jayakrishnan (2022) conducted a review in Type 2 diabetes mellitus. Modern sulfonylureas (e.g., glimepiride, gliclazide XR) vs. Conventional sulfonylureas or other antidiabetic agents was evaluated. Modern sulfonylureas such as glimepiride provide pleiotropic benefits beyond glycemic control in type 2 diabetes, including improved insulin sensitivity, increased adiponectin, and proven cardiovascular safety.
