SUMMARY Data on 50 patients treated surgically for intractable focal epilepsy were analysed. There were 10 hemispherectomies, 32 temporal lobectomies and eight operations to other areas of the brain. The median age at operation was 14 years. 29 patients were found to have mesial temporal sclerosis or sclerotic hemispheres and 17 had alien tissue and one sclerosis with heterotopia. No lesion was found in three cases. The nature of the first seizure was found to relate significantly to pathology. 30 patients had suffered an early convulsion. Between the convulsive insult and the onset of focal epilepsy there was a silent interval, the duration of which was biased by sex and side of lesion. The remaining 20 patients, of whom a highly significant number had alien tissue, had not had an early convulsion before the onset of focal epilepsy. The importance of accurate clinical histories is stressed. Clinical considerations alone predicted the pathology and laterality of the lesion in more than half the series. Regular EEG recordings and repeat scanning were found to be important investigations. Major disorders of behaviour had occurred in 44 patients. As adults, the series were free of severe psychiatric symptoms, but many required years of careful rehabilitation. 31 children had been excluded from normal schools before operation. Postoperatively, many patients continued their education in normal schools. Some required basic remedial teaching, others successfully undertook higher education and training. 49 patients benefited over‐all from neurosurgical treatment. 34 had no more habitual epilepsy at all. Full global gains often took at least five years to achieve. RÉSUMÉ Aspects de développemem chez les enfams trailés par neuro‐chirurgie pour épilepsies focales Les donnees de 50 patients traités chirurgicalement pour épilepsies focales résistant au traitement médical ont été analysées. II tut pratiqué 10 hémispérectomies, 32 lobectomies temporales et huit interventions sur d'autres régions du cerveau. L'âge médian pour les interventions était de 14 ans; il a été trouvé un MTS ou des hémisphères scléreux chez 29 malades, un tissu aberrant chez 17 malades et un cas de sclérose avec hétérotopie. Dans 3 cas, il n'a été noté aucune lésion. La nature de la première crise a été trouvée reliée significativement à la pathologie. 30 patients ont présenté une convulsion précoce. Entre la première manifestation convulsive et le début de l'épilepsie locale, il y a eu un intervalle silencieux dont la durée a varié en fonction du sexe et du côté de la lésion. Les 20 sujets restants chez qui il y avait un nombre hautement significatif de tissu aberrant, n'avaient pas présenté de convulsions précoces avant le début de l'épilepsie focale. L'importance d'une histoire clinique précise est soulignée. Les seules données cliniques prédisent la pathologie et la latéralité de la lésion dans plus de la moitié des cas de la série. Des enregistrements EEG réguliers et des scanners répetés se sont révélés des investigations intéressantes. Des troubles majeurs du comportement sont apparus chez 44 patients. II n'y avait pas dans la série de symptomes psychiatriques sévères chez les adultes mais dans de nombreux cas, plusieurs années de réadaptation soigneuse ont été nécessaires. 31 enfants avaient été exclus des écoles normales avant opération. En post‐opératoire, de nombreux patients ont pu continuer leur éducation dans des écoles normales. Certains ont nécessité une pédagogie curative, d'autres ont entrepris avec succ̀es une éducation et formation supérieures. Globalement, 49 patients ont bénéficié du traitement neuro‐chirurgical, 34 n'ont plus eu du tout de crises épileptiques habituelles. Le gain définitif a souvent demandé cinq ans pour se manifester. ZUSAMMENFASSUNG Entwicklungsaspekte bei fokaler Epilepsie im Kindesalter nach neurochirurgischer Therapie Es wurden 50 Patienten analysiert. die wegen einer unbeinflufibaren fokalen Epilepsie neurochirurgisch behandelt worden waren. Es handelte sich um 10 Hemisphärektomien, 32 Temporallobektomien und acht Operationen anderer Hirnregionen. Das mittlere Operationsalter war 14 Jahre. Bei 29 Patienten fand man eine MTS oder sklerotische Hemisphären, bei 17 Fremdgewebe und bei einem eine Sklerose mit Heterotopie. Bei 3 Fällen konnte kein pathologischer Befund erhoben werden. Die Art des ersten Anfalls zeigte eine signifikante Relation zur Pathologie. 30 Patienten hatten einen frühen Krampfanfall gehabt. Zwischen dem Anfall und dem Beginn der fokalen Epilepsie lag ein anfallsfreies Intervall, dessen Dauer durch Geschlecht und durch die Seite der Lasion bestimmt war. Die restlichen 20 Patienten, von denen bei den meisten Fremdgewebe gefunden worden war, hatten vor Beginn der Epilepsie keinen fruhen Krampfanfall gehabt. Es wird auf die Wichtigkeit einer sehr genauen klinischen Anamnese hingewiesen. Bei mehr als der Hälfte der Fälle konnte die Pathologie und die Lokalisation der Läsion allein durch klinische Erhebungen vorausgesagt werden. Regelmäβige EEG‐Ableitungen und wiederholte Scans erwiesen sich als wichtige Untersuchungen. Bei 44 Patienten traten schwere Verhaltensstörungen auf. Im Erwachsenenalter zeigten die Patienten keine schweren psychiatrischen Symptome mehr, aber viele brauchten viele Jahre sorgfältiger Rehabilitation. 31 Kinder mußten vor der Operation die normalen Schulen verlassen. Postoperativ konnten viele Patienten ihre Ausbildung in regulären Schulen fortsetzen. Einige brauchten eine Sonderschulausbildung, andere absolvierten erfolgreich die höhere Schule und Fachausbildung. 49 Patienten profitierten eindeutig von der neurochirurgischen Behandlung. 34 hatten keine habituelle Epilepsie mehr. Der voile Therapieerfolg wurde jedoch oft erst nach fünf und mehr Jahren erreicht. Habia 10 hemisferectomias, 32 lobectomías temporales y ocho operaciones en otras areas cerebrales. La edad media en el momento de la intervención era de 14 anLos. Se hallo que 29 pacientes tenian MTS o hemisferios escleróticos, 17 tenían un tejido extranLo y uno una esclerosis con heterotopia. En 3 casos no se halló ninguna heterotopia. Se vió que el tipo de la primera crisis estaba en relacion significativa con la anatomía patologica. 30 pacientes habían sufrido una crisis precoz. Entre el insulto epiléptico y el inicio de la epilepsia focal había un periodo de silencio, cuya duración se relacionaba con el sexo y el lado de la lesion. Los 20 pacientes restantes, de los cuales un porcentaje alto tenia tejidos anómalos, no habian tenido una convulsión precoz antes del inicio del la epilepsia focal. Se insiste en la importancia de poseer unas historias bien prccisas. En más de la mitad de los casos las consideracioncs clïnicas por sï solas predecian la anatomia patologica y la lateralidad de la lesiön. Se hallow que los trazados EEG periödicos y los exämenes con scanner eran unas investigaciones importantes. En 44 pacientes existïan alteraciones graves del comportamiento. Al llegar a la edad adulta los casos estaban libres de síntomas psiquiátricos graves, si bien muchos necesitaron años de cuidadosa rehabilitacion. 31 niños habian sido excluidos de escuelas normales antes de la intervenciön. Después de la intervención muchos continuaron su cducacion en escuelas normales. Algunos requirierón educatión especial mientros que otros siguieron con exito una educación elevada. 49 pacientes se beneficiaron en conjunto del tratamiento neuroquirirúrgico. 34 no tuvieron más epilepsia. Los beneficios globales totales tardaron con frecuencia por lo menos cinco afios en ser conseguidos.
