難治性てんかんに対する迷走神経刺激療法は, 比較的手術手技が容易であるが, 迷走神経の正確で迅速な同定が重要である. 迷走神経とその周囲の総頚動脈 (CCA) および内頚静脈 (IJV) の解剖学的位置関係にはバリエーションが多く, 症例によっては迷走神経の同定に難渋することがある. われわれは, 頚部迷走神経の走行をCCA, IJVとの位置関係から5つのタイプに分類した. Type 1 : IJV内側でCCA腹側, Type 2 : IJV内側でCCA外側, Type 3 : IJV内背側でCCA腹側, Type 4 : IJV内背側でCCA外側, Type 5 : CCA背側. 最も頻度が高いのはType 3である. Type 1から5になるにしたがい, 手術手技が煩雑になり, 難易度も高くなる. このような解剖学的バリエーションを念頭に置いてVNS手術に臨むことは, 初心者でも安全で正確な手術を行ううえで効果的である.
