Transcatheter mitral valve replacement can be a feasible and effective salvage therapy for severe complications such as leaflet aneurysm and perforation following edge-to-edge mitral repair.
Offers a transcatheter solution for rare post-repair leaflet perforation; extends Tendyne applicability to complex secondary mitral pathology.
An 81-year-old woman with severe functional mitral regurgitation (MR) secondary to ischemic left ventricular (LV) dysfunction presented to our clinic following recurrent episodes of decompensated heart failure. Her history included a recent anterior myocardial infarction, paroxysmal atrial fibrillation, and severely reduced LV ejection fraction (∼20%).Initial management with edge-to-edge repair successfully reduced a severe functional MR to mild MR (grade 0-1) with a transvalvular gradient of 3.5 mm Hg (Figure 1), leading to symptomatic improvement and good quality of life on guideline-directed medical therapy.
