Background and Objective: Advanced maternal age is associated with various pregnancy risks, including miscarriage, chromosomal abnormalities, and adverse pregnancy outcomes. This study aimed to determine pregnancy outcomes in women over 35 years old who attended Alavi Hospital in Ardabil from 2018 to 2020.Method: This descriptive cross-sectional study was conducted on pregnant women over the age of 35 who attended Alavi Hospital in Ardabil. The women were followed from their first prenatal visit (up to the 8th week of pregnancy) until delivery.Demographic data and pregnancy outcomes were collected from medical records and recorded in a researcher-developed checklist, then analyzed using statistical software. Results: In this study, 300 mothers over 35 years old were evaluated.Their mean age was 39.55 ± 2.46 years, and their mean BMI was 28.57± 2.78.Seven women (2.78%) reported a history of smoking. This was the second pregnancy for 133 women (44.3%), and 31 women (10.3%) had used assisted reproductive technologies.Chromosomal abnormalities in a previous child were reported in 5 cases (1.7%).The most common pregnancy outcomes in this study included cesarean delivery in 104 women (34.7%), gestational diabetes in 21 women (7%), preterm delivery in 19 women (6.3%), and gestational hypertension in 15 women (5%).Stillbirth was reported in one case (0.3%).The most common reason for cesarean delivery was a history of previous cesarean, accounting for 68 cases (60%).Conclusion: Gestational diabetes, preterm delivery, and gestational hypertension were the most common pregnancy outcomes in women over 35 years old. These results highlight the need for specialized prenatal care for mothers of advanced maternal age.
