14 July 2016: The current authors have made no progress with this protocol and the decsion has been made to withdraw it. In 2012 the Colorectal Cancer Cochrane Group reassigned the protocol to the current author team, updating this protocol according to recommendations from the CCCG editorial team. In 2014 the protocol was transferred to the Anaesthesia, Critical and Emergency Care Group. 4 May 2011: Withdrawn from the Cochrane Library as authors can't commit themselves. Available to other interested authors.
