A case is presented of a 53-year-old woman with meta-static lung cancer who did not receive adequate analgesia from intrathecal opioid. The pain was determined to be neuropathic, but a tricyclic antidepressant and gabapentin were inadequate adjuvants. Addition of interpleural bupivacine provided good pain relief for the remainder of the patient's life.
