Urgent double valve replacement achieves clinical recovery in a case of advanced RHD and ADHF.
Why the study?
Advanced rheumatic heart disease with end-stage sequelae is rarely seen in the United States, involving complex pathological interactions when requiring cardiac bypass.
Case Report (n=1)
No
Urgent double valve replacement can be successfully performed in patients with advanced rheumatic heart disease and severe biatrial dilation presenting with acute decompensated heart failure, despite a high calculated mortality risk.
Advanced rheumatic heart disease (RHD) with end stage sequelae is rarely seen in the United States. We report a complex case of a patient with advanced RHD complicated by Tricuspid stenosis, tricuspid regurgitation, mitral stenosis, and pulmonary hypertension in the setting of acute heart failure who undergoes urgent double valve replacement surgery. There are many notable complex interactions of the pathologies in patients with RHD who need to go on cardiac bypass.

Abraham et al. (2018) conducted a case report in Severe Rheumatic Heart Disease and Acute Decompensated Heart Failure (n=1). Double valve replacement was evaluated on Clinical recovery. Urgent double valve replacement was successfully performed in a 69-year-old female with advanced rheumatic heart disease and acute decompensated heart failure, resulting in gradual recovery.
