The Simultaneous Kissing Stent (SKS) technique can be a feasible and effective approach for treating unprotected distal LMCA bifurcation lesions in patients who refuse CABG, particularly in high-risk scenarios like left dominant circulation.
Left main coronary artery stenosis is the most challenging lesion in patients with acute coronary syndromes. Since the amount of myocardium at risk is very high, the patient is often in cardiogenic shock, and the risk of death is high and even more so in a left dominant coronary system.

HS et al. (2015) conducted a case report in Unprotected Distal LMCA stenosis with acute coronary syndrome (n=1). Simultaneous Kissing Stent (SKS) technique was evaluated on Procedural success (residual stenosis). Double stenting of the unprotected left main coronary artery with bifurcation disease using the simultaneous kissing stent technique was successfully performed with no residual stenosis.
