Objective: Comparision of the outcomes of Kerr incision extension in transverse or cephalocaudal direction in primary cesarean operations. Material and Methods: Our prospective randomized controlled study was performed between January 2015 and December 2015 in our hospital. The patients who had no previous cesarean operations, no placental invasion disorders or coagulation problems and older than 34 gestation weeks were divided into two groups according to the extension techniques. Patients whose Kerr incision was extended transversely were included in Group 1 (n=189) and those extended in cephalocaudal direction were Group 2 (n=186). Demographic data, obstetrical and medical history, fetal weights, APGAR scores, complications during operation and blood test results before and after the operation were recorded and maternal blood loss was calculated. Results: Complications like uterine vessel injury, bladder injury or uterine atony during operations were similar in both groups. Hemoglobin decrease in Group 1 was 1.58±0.06 g/dL and hematocrit decrease was 4.60±0.19%, where as it was 1.65±0.07 g/dL and 4.97±0.21% in Group 2. No statistical differences were determined between groups in preoperative and postoperative hemoglobin and hematocrit values. Blood transfusion rates were similar in both groups. Conclusion: We found no difference in intraoperative and postoperative outcomes between two techniques.
