Intraligamental leiomyoma refers to the lateral growth of a uterine leiomyoma between the folds of the broad ligament. Preoperative imaging is important in surgical planning. We report the case of a 36-year-old woman diagnosed with a large intraligamental leiomyoma. Magnetic resonance imaging findings of T2 hypointensity and bridging vessel signs were suggestive of uterine leiomyoma. Other imaging findings that include shape deformity, low uterine attachment, superior and medial displacement of the right ovary, and separation of the round ligament and the uterine artery can help in identifying the intraligamental location of the leiomyoma.
