The ovarian veins are the most frequently involved veins in puerperal pelvic venous thrombosis (Allan et al., 1975). We present a case with extension of lithrombus from the right ovarian vein into the inferior vena cava. A 28-year-old gravida 3, para 3 had a vaginal delivery followed by manual removal of the placenta. Twenty-four hours later she developed fever and right lower quadrant pain and tenderness. Appendicitis was suspected and when rebound tenderness was found on the 4th postpartum day, a laporotomy was performed.
