A modified, isotopic dilution technique for assaying serum vitamin B., levels Is described. This Is shown to give rapid reproducible and clinically useful results which are not affected by the presence of therapeutic drugs. The range obtained with 125 normal subjects and hospital control patients was 250 to 1,200 pg/ml, with a mean of 547 pg/ml. Twenty-one patients with untreated pernicious anæmia gave results ranging from 42 to 246 pg/ml, with a mean of 113 pg/ml.
