Key result
CT-guided LAAO successfully closes a 13 mm residual stump, demonstrating a viable alternative to TEE.
Why the study?
Residual stump formation of 10 mm or more is a recognized complication after surgical LAAO with AtriClip, where TEE has been the standard imaging modality.
Case Report (n=1)
CT imaging is a viable and less invasive alternative to transesophageal echocardiography for preprocedural planning and follow-up in percutaneous closure of residual postsurgical LAA stumps.
May support CT-guided LAAO in residual stumps; leaves open need for prospective trials before replacing TEE.
Percutaneous and surgical approaches have been developed for left atrial appendage occlusion. Residual stump formation with a depth of ≥ 10 mm is a recognized complication after surgical LAAO using AtriClip (Atricure, Mason, Ohio). The standard imaging modality for preprocedural planning and postprocedural follow up during these procedures has been transesophageal echocardiography. The current case describes successful closure of a residual stump, measuring 13 mm in depth, using percutaneous LAAO, with CT imaging utilized for preprocedural planning and postprocedural follow up. Accordingly, this case demonstrates that CT imaging is a viable, convenient, and less invasive alternative for these situations.

Saeed et al. (2025) conducted a case report in Residual postsurgical left atrial appendage closure stump (n=1). Percutaneous LAAO using Watchman device guided by CT planning was evaluated on Closure of residual stump. Percutaneous LAAO using the Watchman device guided by CT imaging successfully closed a 13 mm residual stump, demonstrating CT as a viable alternative to transesophageal echocardiography.
