An unusual case of metastases to a number of extraocular muscles (EOMs) from an adenocarcinoma of the stomach is reported. In a review (1) of 227 patients with metastases to the eye and orbit, the ratio of ocular to orbital involvement was 7:1. Among all orbital tumors, those due to direct metastases (i.e., not by contiguous extension from adjacent structures) accounted for only 0.4 to 10.5%. Discrete metastatic involvement of one or more EOMs is an even rarer event, with a literature consisting exclusively of case reports (2). In this case there was symmetrical involvement of both lateral rectus muscles and the left superior oblique muscle.
