Key result
Diabetic patients, especially those requiring insulin, had a significantly higher risk of adverse events at 3 years compared to nondiabetic patients after drug-eluting stent implantation.
Why the study?
Whether currently available drug-eluting stents have similar safety and efficacy in diabetic and nondiabetic patients remains debated.
Does drug-eluting stent implantation yield similar 3-year outcomes in diabetic versus nondiabetic patients with coronary artery disease?
Observational
Yes
Does drug-eluting stent implantation yield similar 3-year outcomes in diabetic versus nondiabetic patients with coronary artery disease?
Diabetic patients, particularly those on insulin, continue to have a significantly higher risk of adverse events after DES implantation compared to nondiabetics, highlighting the need for improved DES performance in this population.
Diabetics, especially insulin-treated, show higher 3-year events after DES; supports targeted device trials but should not yet alter practice.
Diabetic patients are frequently affected by coronary artery disease (CAD) and are at increased risk of CAD-related adverse events, even after drug-eluting stent (DES) implantation. If currently available DES have similar safety and efficacy in diabetic and nondiabetic patients is still debated. This prospective, multicenter registry showed similar 3-year outcome in patients undergoing different DES implantation, although diabetic patients, especially those requiring insulin treatment, had significantly higher risk of adverse events than nondiabetic patients. Specific efforts to improve the performance of DES in diabetic patients are mandatory to adequately address the unsolved issue of diabetic patients affected by CAD.

Chiarito et al. (2020) conducted an observational in Coronary artery disease. Diabetes vs. Nondiabetic patients was evaluated on Adverse events. Diabetic patients, especially those requiring insulin, had a significantly higher risk of adverse events at 3 years compared to nondiabetic patients after drug-eluting stent implantation.
