The clinical, hematologic, and serologic features, before and after splenectomy, of 15 patients with Felty's syndrome are described. Follow-up periods ranged from 1 to 13½ years. Splenectomy relieved the neutropenia in 66% of the patients, with no late relapses. The frequency of infections was markedly decreased in most of the patients who had a return of the neutrophil count to normal after splenectomy. The indications and contraindications for splenectomy are discussed.
