血管肉腫の頻度は我が国の皮膚悪性腫瘍のなかの約1%程度で稀な腫瘍であるが,生物学的悪性度は非常に高く予後は悪い。今までにみられる報告も,また我々の施設の治療成績においても5年生存率はおおむね10%台である。それにもかかわらず,血管肉腫に対する診療体系は整っておらず,診療ガイドラインも存在しない。一般に固形がんの治療原則は十分な手術であり,血管肉腫の場合も局所制御のためには外科的完全切除と放射線治療が有効であると考えられる。遠隔転移に対しては全身化学療法が治療の主体となり,抗がん剤単剤ではpaclitaxel,docetaxelが,多剤併用療法ではMAID(Mesna+Doxorubicin+Ifosfamide+Dacarbazine)療法が高い奏効率を示す。今後は血管肉腫に対するquality of lifeや予後の改善を目的とした治療方法の開発が望まれる。
