A posterior midline approach for the surgical treatment of chronic calcific Achilles insertional tendonitis is a safe and reliable procedure. It respects the angisomal blood supply to the Achilles tendon and allows direct exposure to the primary pathology, which forms centrally in the tendon. When coupled with the resection of the Haglund deformity and the Strayer release at the musculotendinous junction, this technique provides reproducible and reliable results.
