Discharge systolic blood pressure demonstrated a -shaped association with cardiovascular mortality and heart transplantation in patients hospitalized for decompensated heart failure.
Why the study?
The association between SBP and mortality in HF is paradoxical, and baseline SBP has been assessed at different time points across prior studies.
What is the association of systolic blood pressure at admission and discharge with postdischarge cardiovascular death and heart transplantation in patients hospitalized for decompensated heart failure?
Cohort

What is the association of systolic blood pressure at admission and discharge with postdischarge cardiovascular death and heart transplantation in patients hospitalized for decompensated heart failure?
There is a non-linear association between discharge systolic blood pressure and postdischarge cardiovascular mortality or heart transplantation in patients with decompensated heart failure.
May warrant cautious discharge BP titration in decompensated HF; leaves open optimal targets for prospective validation.
BACKGROUND: The association of systolic blood pressure (SBP) with mortality in heart failure (HF) patients is paradoxical, and the time points of baseline SBP are also different across prior studies. We hypothesized that the levels of SBP at admission and at discharge had different associations with postdischarge events. METHODS: The study population included patients hospitalized for decompensated HF in the Heart Failure Center of Fuwai Hospital from January 1, 2009 to December 31, 2014. The primary outcome was a composite of cardiovascular (CV) death and heart transplantation. Multivariate Cox proportional-hazards and restricted cubic spline analyses were used to assess the relationships between SBP at different time points and outcomes. The results of restricted cubic spline analysis indicated similar associations. CONCLUSIONS: -shaped association between discharge SBP and CV mortality/heart transplantation.

Zhang et al. (2019) conducted a cohort in Decompensated heart failure. Systolic blood pressure at admission and discharge was evaluated on Composite of cardiovascular (CV) death and heart transplantation (p=0.291). Discharge systolic blood pressure demonstrated a -shaped association with cardiovascular mortality and heart transplantation in patients hospitalized for decompensated heart failure.
