Urgent mitral valve repair rapidly resolves acute severe MR presenting as unilateral pulmonary edema.
Case Report (n=1)
No
Acute severe mitral regurgitation should be considered in the differential diagnosis of patients presenting with atypical unilateral pulmonary infiltrates and rapid clinical deterioration, as early echocardiography is crucial for timely surgical intervention.
Unilateral edema in acute MR may respond to urgent repair; single case leaves open need for confirmatory studies.
A 40-year-old man presented with right-sided pulmonary infiltrates mimicking pneumonia. Imaging and transesophageal echocardiography revealed unilateral pulmonary edema (UPE) from acute severe mitral regurgitation (MR) due to chordal rupture. Unilateral infiltrates with rapid deterioration warrant early echocardiographic evaluation.

Inoue et al. (2026) conducted a case report in Unilateral pulmonary edema mimicking pneumonia due to acute severe mitral regurgitation (n=1). Urgent mitral valve repair was evaluated on Clinical recovery and resolution of pulmonary infiltrates. Urgent mitral valve repair resulted in rapid respiratory improvement and complete resolution of pulmonary infiltrates in a patient with acute severe mitral regurgitation presenting as unilateral pulmonary edema.
