Circulatory collapse and cardiorespiratory arrest occurred in a 40-year-old woman following massive propranolol ingestion. Conventional therapy with sympathomimetic drugs and cardiac pacing were ineffective in restoring blood pressure; however, high-dose isoproterenol (200 μg per minute) and glucagon rapidly brought her blood pressure under control. Despite an initial protracted period of drug-resistant hypotension requiring continuous cardiopulmonary resuscitation, the patient recovered completely and left the hospital within three days. Thus, high-dose isoproterenol and glucagon may be lifesaving in cases of severe beta blocker overdose.
