DEFIANCE trial tests ClotTriever mechanical thrombectomy against anticoagulation alone in ~300 iliofemoral DVT patients.
Why the study?
Mechanical thrombectomy-based procedures for iliofemoral DVT have not yet been compared with standard-of-care anticoagulation therapy in randomized studies.
Does mechanical thrombectomy improve clinical outcomes and reduce postthrombotic syndrome severity compared to anticoagulation alone in patients with unilateral iliofemoral DVT?
RCT (n=300)

No takes yet. Share an insight, caveat, or question.
The DEFIANCE study will be the first randomized controlled trial to evaluate mechanical thrombectomy versus standard anticoagulation alone for the treatment of iliofemoral deep vein thrombosis.
Abramowitz et al. (2024) conducted an RCT in iliofemoral deep vein thrombosis (n=300). Mechanical thrombectomy with the ClotTriever System vs. Anticoagulation alone was evaluated on Composite outcome structured as a hierarchal win ratio of treatment failure or therapy escalation and PTS severity at 6 months. Mechanical thrombectomy with the ClotTriever System is being evaluated against anticoagulation alone for iliofemoral DVT in the ongoing DEFIANCE trial of approximately 300 patients.
